The healthloc.xls and .csv files contain the following variables
Note the .csv file does not have the variable names at the top of the columns
since Mplus can only read in the data not the names of variables. 

Column1 - idnumber
Column2 - age (in years)
Column3 - gender (1 is male, 2 is female)
Column4-Column10 - the responses on a 4-point Likert scale to the 
following 7 questions

"Avoidill"  If I take care of myself, I can avoid illness.
"Ivedone"   Whenever I get sick it is because of something I've done or not done.
"Hlthfort"  Good health is largely a matter of good fortune.
"Nomatter"  No matter what I do, if I am going to get sick I will get sick.
"Strange"   There are so many strange diseases around that you can never know how 
            or when you might pick one up.
"Pllucky"   People who never get sick are just plain lucky.
"Response"  I am directly responsible for my health.
